31 October 2013 I am about to change my current surname to my actuall family name. From where to get afidavit? Is it a stamp paper or typed on blank paper? On what conditions does notary attests such affidavit? Who are the witnesses? What's the further procedure?

31 October 2013 It will be on stamp paper and will be notarised.

You will also publish the same in newspaper one in English and second in regional language.

There after you make application for publish the name in official gazzet for acceptance of your change name with other regulatory authority.

They will ask copy of gazette in which your current name is published.
