Cancellation of tan

I have a one tan number and i want to cancel the TAN number i tried a lot to search the form but i didn't find the form so please anybody tell me from where i can download the form.

You have to make an application with the jurisdictional A.O (TDS), on a plain paper, for cancellation of TAN and state reasons for such a request.
