01 May 2012 you have to register your TAN no online (www.tin-nsdl.com) after that you will get procedure how to revise the TDS return.

01 May 2012 you need fvu file for said qtr (how to send request, procedure given in your tan login account) and also need a software to process to rectify the said entries and create revised tds return (for software you may go to your concern ca firm or interent).
