My company name has been changed. I want to amend in Service Tax Registration of Name change. In this regard, i have requirement board resolution format. So, please help in this matter.

No need to pass Board resolution.
You only submit new COI with shareholders resolution already passed with Covering to Service Tax Department.

Ok but Form 23 also submit to Service Tax Department and what mean of covering?
