01. Covering letter stating that internal audit report is attached
02. Internal audit report : address it to the one who appointed you...say company, society etc.
03. Make the sheet containing the observations made by you. Write down the relevance for the same in next column. Keep a column blank for the action taken by the AUDITEE.
