I have completed 1yr & 2months in a firm and due to health issues i din go to office for 1 month and now because of health issues i want to terminate the articleship. Can anyone pls guide me with this regards.

Yes You can termination
as your Articleship period exceeded one year you should write a letter and get it signed by your principal and send the same to ICAI for permission.
Once the permission is confirmed by ICAI then you can fill up form 109

Sir, Institute has given 3months criteria for medical reasons, whether this condition will affect my termination? ?

yes. you need to submit medical documents (generally from a government hospital) to certify your medical condition that warrants a break for more than 3 months.
