While filing form 5 we have to file Altered MOA,what will be the date and who
shall sign the altered MOA, who shall be
the witness to it ,what will be the details on share subcribed column.
If possibe can u give a format of last page of altered MOA.

CS or any director of the company can sign altered MOA to be attached with form-5.
First 3-4 pages and last page of MOA Table is to be signed in case of MOA including altered capital clause.
First last page of AOA Table is to be signed in case of AOA.
No date is required to be put on MOA. ROC will presume the date of GM for signing purpose.
