Please recommend what kind of agreement to be drafted???

Agreement based on the nature of relationship incase professional contract only you can draft professional retainership agreement,incase salaried case,just employment agreement enough

If the doctor is hiring the OT then lease agreement and if the doctor is using the operation theater then the agreement should be business arrangement. What actually you want is not clear. Kindly brief accordingly. If the doctor want to own the OT then it is Sale agreement and so on.
