Iam working in Pvt Ltd company, we have paid service tax wrong accounting code ,
my quation is change the accounting code as per actuval, is there any affidavit format or any other process

In your case as per one of the board circular you need not to pay tax again. In other words, if you have deposited tax in wrong accounting code then you need not to deposit the same again. You can intimate the same to range superintendence by writing a simple letter stating above fact.
