4. Copy of Registration Certificate granted u/s. 12-A(a) by Commissioner of Income-Tax, bearing Registration No..

5. Copy of Registration Certificate granted by Deputy Charity Commissioner of (City) Dated.. bearing Registration No..

6. Copy of the Trust Deed (English).

7. Notes on activities of the Trust.

8. Names, addresses and PAN of the Trustees of the Trust.

9. Copy of Acknowledgements of Income Tax Return filed for last three Assessment years.

10. Copy of Audited Annual Accounts for the last three years alongwith the audit reports.
