what is the format of working diary ?
how it should b maintained
can it b a loose file or a bound book
is there any fixed format given by icai
or simply how working diary should appear

it should consist of Name, date, particulars of work done,timming for in and out.

Yes,
as per icai regulations it is the responsibility of student to maintain work record,
however you can prepare that three months also as per your knowledge and get it signed by your boss
so please maintain work diary in some specified format which shows,
date
work done
work given by
and time of work started
closing time of work started
and if any excess work is done it can be mentioned ..
