How to maintain a work dairy and is there any prescribed format?

there is no prescribed format
u just need to write the kind of work u have performed during the day.....
it must contain :
name of the assessee and the article clerk
date , nature of work performed, queries if any, time taken and signature

As per my knowledge, the format is yet to be prescribed by ICAI for work diary, but right now you must follow what Devanshi has suggested.
