1.what are the attachment a in dir 12 for the appointment of CS. 2. does name in the form can be filled in initials for cs appointment as my surname is in initials in cs membership .

As per my opinion Board resolution for appointment of CS is must as an attachment. You may also attach your proof of identity and address proof as an additional attachment.
