now you can re-register with other ca and submit form 103 no need to wait for icai confirmation
During first first year of transfer you only submit Form-109 with Institute counter signed by your principal.

if you forgot that time send it with re-registration documents(form 103) along with a covering letter says ,that i forgot that time,,please acknowledge...
You will send such card at the time of re registration.
