19 June 2014 I was completed one nad half year articleship i want to take transfer now, i read procedure but i didnt got that discontinuance period for minimum 3 months on medial grounds (on the production of medical certificate) and what is format for getting permision from institute

19 June 2014 it means that the illness should require atleast 2 months of rest and during this period you wont be able to serve articleship.

there is no format...just write your application on a plain paper.
