Pls tell if one Collected Service tax on Rs 5 laxs
does he liable to pay tax to govt
if assessee has taken exemption previous return

Ideally service tax is not applicable on your turnover.
however you had collected same form customer then your required to pay same to government.
1) Apply for registration.
2) file six monthly ST 3 return.
