I would like to know that is there any specified format for the declaration for claiming SSI exemption benefit under notification no. 8/2003 C.E.?

02 May 2013 For availing exemption u/n no. 8/2003 you are not required to file any declaration.

Only when you do not want to avail exemption under the said notification then you have to file a declaration.
