if a company shifted its registered office 4 month within the local limit..and not informed the roc yet what will be the penalty

Not much penalty will be there. You should file form INC 22 within 30 days of passing board resolution. When you upload form INC 22, challan will be generated with additional fees.
No separate penalty will be there.
