i want fill up the st-1 form for an individual.
"In the name of applicant" portion what will be there the name of the proprietor or trade name? if the name of the proprietor i mention there then where i will mention the trade name?

Name of applicant will be Trade Name. And you should write Name of the Proprietor where details of proprietor are asked.

Please apply by proprietor name as serice tax registration in case of proprietor is always by the name of proprietor who may have multiple trade names & can not apply multiple time for registration.
Also under service tax regulations proprietor is considered as One Assessee for any number of services being provided by him.
Also registration no would be PAN No of proprietor +ST001
