Client having registered office of the company in Mumbai, wants to apply for service tax registration from indore, can we apply from indore?

You can apply from indore, provided you have an official address at indore and the output service will have to be done from indore and billing will have to be done from your indore office.

Provision of services and billing will be from indore, but will that require any additional documents for registration?
and will it be a problem for registration if address is different on Incorporation certificate and Rent agreement?

You have not informed whether you are an individual or firm or private ltd co etc.,
Any how, there is no problem in getting RC in indore. You need to provide all particulars like, address proof, Telephone no/EB bill/Municipal tax receipt of the premises in that address, Firm pan number, rental agreement, Parner's address proof, pan card number, VAT RC of that premises, partnership deed or memorandum/articles of association if it is company, etc., But the address proof shown in the rental agreement should tally.
