Sir/ madam i need a sample letter of getting approval from icai requesting for termination of articleship on the ground of health issue.and am just entered my firSt year. Kindly help

I, ..........having registration no.............doing article ship under............., Chartered Accountant, is willing to take transfer on the ground of my marriage which is going to be held in next months ........ I also want to request that after marriage i will be unable to continue article ship with my existing principal due to....(you can mentioned your any reason.
So requesting you to kindly consider my transfer application and give approval. You are hereby requested to give approval.
