Revision of form 5

Dear Members,
In an EGM of a co. both increase in Authorised Share capital and sub division of shares was approved but when Form 5 for the same was filed only increase in Authorised Share Capital was considered, we skipped to consider the sub division of share.
Now,
1. Is there any option to revise Form 5.
2. If not then how can we rectify this mistake.

what is sub division you are talking .
It is pre and equity shares division.

It is sub division of face value of equity share from rs.100 per share to Rs. 10 per share.
