which form should be use for filing of revised Service tax return for (Apr-June 2012)

ST-3 is to be used for revise the return (as there is only on form for service tax return).
But revision should be within 90days for the date of filling of original return.
But revision should be within 90days for the date of filling of original return.

ST-3 for Apr-jun 12 cannot be filed now as 90 days is already lapsed.
