Dear Sir,
I have taken transfer and filled form 109 but due to some miscommunication transfer date has been wrongly dated and I want to revise the date in from 109.
Can I do so?

As per my suggestion you should visit your nearest branch of ICAI with all necessary documents. I am sure they will handle your request and provide you the opportunity to rectify the document.
