1.SERVICE TAX REGISTRATION CERTIFICATE COPY ISSUED BY THE CENTRAL EXCISE DEPT. TO THE SERVICE PROVIDER(MUST)
2.EVIDENCE OF PAYMENT OF SERVICE TAX COLLECTED BY THE SERVICE PROVIDER (OPTIONAL).
