Reformatted TAN No.means :- TAN Number alloted by Income tax dept. after any changes or corrections were made in the data associated with earlier TAN.

For online uploading of TDS returns you have to do registration at tin-nsdl.
once you have register your TAN at tin-nsdl for online uploading TDS returns. Your TAN number will automatically become reformatted TAN.
