What is the limit above which i need to quote PAN in case of cash withdrawals from my personal account?

In case of withdrawl , No need to mention PAN for any amount.

The applicants (whether customers or not) for Cash transactions for amount
exceeding Rs.50,000/- should affix Permanent Account Number (PAN) on the
application.
