this is very urgent.How do one apply for profession tax enrollment certificate?

19 November 2010 Professional tax comes under State Act. (In Karnataka)Apply in Form 1 & 2 to the concerned PT officer.
In other state the concerned form.

doc to be produced

rental agreement
2 passport size of the proprietor/director/partner
deed in case of firm
MOA/AOA in case of company
salary statement from which month the PT is payble.
