ESI AND PF REGISTRATION DOCUMENTS
1. ESI- You have to address your application to The Officer, Employees’ State Insurance
Corporation, of your city with the following documents requesting for your establishment to be covered under ESI Act and For, PF you have to write to The Regional Provident Fund Commissioner
Regional Provident Fund of your city with the following documents in duplicate:
From – 01 (ESIC)
2. Trade License
3. Certificate of Incorporation
4. Copy of Pan Card of both company and directors or proprietor.
5. Owners Residential Address Proof & Company Address proof.
6. Bank Statement
7. Attendance Register
8. Salary Register
9. Memorandum & Articles of Association
10. Audited/ Unaudited Financial (if any)
11. Consent signature of staff (in case of PF)
