WHAT SHOULD BE CONSIDERED IN SALARY FOR PF AND ESI?

For PF Salary refers to Basic Salary limit Rs.15000/-. ESI is Gross salary limit Rs.15000/-

what is the difference between NET SALARY AND TAKE HOME SALARY.. This is my salary structure.
GROSS SALARY=Basic+HRA+Conveyance+Medical+TELEPHONE+LTA+OTHERS.
PF 12%(EMPLOYER/EMPLOYEE) = 12% OF BASIC .....................(Correct or not?)
ESIC 4.75% = (Basic+HRA+Conveyance+Medical+LTA)*4.75%....................(Correct or not?)
ESIC 1.75% = (Basic+HRA+Conveyance+Medical+LTA)*1.75%....................(Correct or not?)
Please advice at earliest..

Esi not applicable your gross salary exceeds Rs.15000 /- limit PF 12% Basic. Limit Rs. 15000/- employer 12% of Basic
