Not received tds refund of ay 2001-02 to till date

My wife is salary person and she has Filed ITR -2D for the AY2001-02 with refund Rs.10268/- on dated 12-07-01 Ack. no. 1854 to 15(1) (L to Z) New Delhi. But after marriage she is filing her return to Ghaziabad. Request you to please guide us from where we can get this income tax refund. what is the procedure or any refund claim form need to be submit. Please guide us in which income tax authority we can approach. please provide us the information in details.
