Missing information on form 103

I have forgotten to fill up part 17 of form 103. They have sent me a mail regarding this! And asked me to write a letter from the principal to furnish the information! Can anyone give me the format of the letter what to write

In the form 103 of Sudipa Saha forwarded to the Institute on ,,,,February 2018,
column 17 is not filled due to an omission. How the particulars are given below
in seriatim.

I now request that the mistake may be condoned being an honest omission from my part and the article ship may be granted to sudipa Saha with effect from the date
of commencement mentioned in column 11 of form 103.
