If C form misplace by courier company or Post office at the time of transit, how can we get or what will be precedure to get same or new c form by sales tax department?

An FIR is required to be made to the jurisdictional Police Station of office of the courier or Post Office from where it is lost in Transit.
An advertisement is required to be given in the local news paer about the loss of "C" form.
Apply to the Assessing Authority who has issued the "C" form to cancell the same. Also apply seprately for the issue of fresh "C" form.
