Please help in getting the required indemnity bond for loss of road permit.

19 September 2012 make it on INR500 stamp paper stating that I indemnify the president of India through its RTO...so and so for the loss if any caused or to be caused or has caused for the misplace of route permit...(Give the details of route permit in detail)
