25 May 2016 Dear Sir,
My client is lost his pen card so i want to apply for his new pen card whether in form no : 49 or change in request for new form . if we apply through second option then we have not photo copy of original pen card so how to obtain photo state of orginal pen card. But in above case i have not a original PAN no. so how can i obtain original pan no.

If you have PA No. then you can make application for duplicate PAN card in the second option as mentioned you (i.e. correction in PAN). you don't need photocopy for the same just PA number is required along with proof of identification (along with DOB) and address.

25 May 2016 you apply on Duplicate PAN form and attach required documents and deposit with NSDL nad you get PAN in 15 days.
