Huf pan registrations

I want to see and check 26AS i.e TDS and Tax credit to my HUF A/c. But the registration process ask three different name, such as first name, middle name,last name,and what will date of creation if PAN card shows as "Ancestral"

Please fill up the name as written on PAN Card in "Last Name " Only.
Write Date of Incorporation as 01/01/0001.
Write Date of Incorporation as 01/01/0001.
