My IT Refund has gone back due to wrong bank details.please suggest me to collect the refund amount

Check the status on income tax site. If it is showing FAILURE, send a letter to CPC.
If you have filed manual/ physical return, contact the concerned Assessing Officer.

No format as such. Make a request letter giving the details of bank account number and e mail id.

IF YOU HAVE FILED ELECTRONICALLY THEN OBVIOUSLY YOU HAVE A LOGIN-ID PASSWORD, JUST LOGIN AND FILL A RESPONSE SHEET SUBMIT IT TO DEPARTMENT WITHIN 21 DAYS YOU WILL GET REFUND(PROBABLY).
