Health care services are exempt from gst but lens which are used for surgery are taxable @18%. So, this service is taxable or exempt from gst?

This service is exempt but lens is taxable at 18%.

Then doctor is required to take gst registration only for lens provided even is primary service is exempt.

No registration is required as he is purchasing the lens from registered GST dealers by paying GST.
In case he sells in excess of purchase price and threshold crossed GST registration is required.

And one more is questions is, lens provided with Sergey is treated as composite supply?

Health services exempt from GST. So it will not fall under composite supply.
