Format of Condonation Letter along with FORM 108

The submission of Form 108 relating to Certificate of Service under Articles has got delayed.
I would have to pay a condonation fee of Rs. 300 along with a condonation letter because of the delay. I am a little confused regarding the format of the same.

I appreciate your fast response.
But I have already seen all the file formats given in the above link, none of them relate to the Condonation Letter for Form 108. In fact, none of them are even addressed to the Institute.

15 December 2010 No format needed. Just a simple letter to the institue expalining the reason for delay in submitting the form.
