call back the form from vendor and rectify under seal and signature of owner

Call for the old C form and cancel the same and issue a fresh one. Send intimation tot he Sales tax department for cancellation of the old form.
If you do not want to do so then call for the old form and cut only entry put the initial with rubber stamp and send it across to the supplier.
