Form 6 for cop

29 September 2014 Dear sir,

I am interested to start my own practice in ca. whether i need to file both form 2 and form 6. or only form 6.

form 2 is for the membership
form 6 is for cop

please provide both forms and clarify whether both is required or only one form i.e. form 6.

03 October 2014 if you are applying for the membership for the first time (ie you have never been a member before), then form 6 is not required.
