help with with points11(b) to 19. what should i fill in these points.

- Under 11 (a) mention "Yes" left 11(b) and (c)
- Left point no. 12, 13, 14 Blank
- Under point 15 mention "No"
- Left point no. 16,17 Blank
- Under point 18 mention "No"
- Left point no. 19 blank

03 March 2017 Point 13 and 14, check whether you fall in any of the category mentioned and fill it up accordingly, otherwise strike out those points as not applicable. As mentioned above by expert, all places where it is suggested to leave it as blank, strike it as Not applicable.

in the id form, the space gvn for photos, should we paste dere or not??
because in the starting line they mentioned that "bring photos with your name behind it"
