My previous company not intrest to give form 12B. Can I fill myself and submit to new company? If yes, how can I fill those information?
I have joined witn new company on Oct 04 2010. I have Form 16 for the year (2009 to 2010) issued by my previous company in the month of march 2010.

Form No.12B has to be given to the new employer by the employee and not the then employer. Form no 16 for the year 2009-10 is irrelevant since you joined with the new employer since that year ie., on 4th October,2010. You may therefore, down load form 12B from www.incometaxindia.gov.in and fill in the particulars of your income from the then employer relating to the period from 01/04/2010 to the date of resignation and submit to the new employer for determining the TDS on salary.
