Facing problem while filling form 2 for membership

I want a clarity in point no 19, whether in that point I have to write my wish trade name or whether in which firm I want to do practice, because I m joining with my friend in practice, so I have to write that firm name or some other name nd one more point that whether i have to paste my photograph in this form or not

you wont fill point 19 at all. once you get the COP, your friend will file form 18 with ICAI
