Extension in articleship period

In case of extension of articleship period if there is a break in artlicle service period, then what will be the documents that need to be executed under the same employer. please help.......

Sir, I had not taken termination. I had filled form 108 for completion of article service. But extension of Nine(09) days has been given by institute. Now after my exams are over i want to serve the extension period under the same principal. Should i again fill form 103...?
