how to pay monthly contribution in offline mode. and how to generate challan for it

Esic payment to be made only online From March -2015.
if you want to generate offline than you have to file monthly contribution than submit , choose payment mode by cheque submit in notified Bank. but now ESI Payment ( mandatory) accepted only ONLINE
