E-filing for company incorporation

I e-filed forms for incorporation of company and MCA required some corrections as below.
"IA code may require corrn., MD is not named in AOA. section 15(1)(c) and 33(1)(c) were not complied with. tax receipt issued by the civic body to be attached". Please can any one explain me what they required.

a) Industrial activity code (clause 7 of e-form 1) may be corrected. Code may be selected from the list given based on the main object of the company.
b) MD'S name to be included in the AOA under the clause of DIRECTORS.
c) 15(c) - last page of MOA and AOA must be written in the own hand writing of the subscribers and their signature must be attested. The attesting person has to write his/her name, address, description and occupation in his/her own hand writing.

Sir, Thanks for reply. Our activities are related to HR and IT like training, placements, software development and web designing. for this which IA code will be applicable. And what about Sec.33(1)(c) and tax receipt issued by the civic body to be attached.

d) 33(1)(c) - agreement between company and any individual to be appointed as managing director may be uploaded, If no such agreement, a declaration to that effect shall be uploaded.
e) tax receipt of the building in which registered office is situated shall be attached with e-form 18.

Sir, I mentioned all the directors name in AoA but not mention whom MD and Directors. Whether we want to mention who is MD. We want to get consent from MD only or from other directors also.
