Duplicate PAN card

13 June 2011 one of my client had lost his pan card in 2008. he had not informed us reg this earlier. can we ask for a reprint of pan card as suggesteed in the arlier resolved query by submitting copy of old pan card, identity proof & address proof? or is it that we have to fill in form 49A?

16 July 2011 now the problem is that the client had shifted to thane from kalyan in the year 2008 and the pan card was applied from the KYN. so now if we submit the request for new pan card, the identity n resident proofs will have the new address. then he will have his address changed in the pan records... now the query is his IT ward no. will also change from kalyan to thane?
