02 February 2011 Apply in form called as " request for new pan card or/ any change or correction in pan data". Form is available in www.income taxindia.gov.in or in the designated offices of UTI / NSDL. Fee payable is Rs.94/- per application.

MR MOHAN APPLY IN NEW FORM AFTER PROVIDING ADDRESS PROOF WITH TIN FACILITATION CENTRE OR UTI FACILITATION CENTRE.

In the original PAn card, the address given was the office address.Now when I apply for duplicate, can i give my residential address along with the id proofs?

you can fill up your residential address but before apply NSDL , provide the election card or ration card or prescribed documents listed by department as a proof .
