Can any tell me the procedure for taking duplicate PAN Card an my original got lost.

Apply in form called as " request for new pan card or/ any change or correction in pan data". Form is available in www.income taxindia.gov.in or in the designated offices of UTI / NSDL. Fee payable is Rs.94/- per application.

Dear Sir,
In the original PAn card, the address given was the office address.Now when I apply for duplicate, can i give my residential address along with the id proofs?

you can fill up your residential address but before apply NSDL , provide the election card or ration card or prescribed documents listed by department as a proof .
