Please can anyone tell me what is the procedure for applying duplicate PAN
I had lost it. Need it urgently.

You need to apply in form 49A. The same form for applying new PAN.

The FAQ at NSDL site says :
"Those who have a PAN but have not received a PAN card or have lost their PAN card or wish to have a new PAN card with existing details, can fill the form for Request for New PAN Card or/and Changes or Correction in PAN data. Applicant is required to fill all the columns in the form but should not tick any of the boxes on the left margin."
